FEES AND HEALTH PLANS

   FEES

   •  individual session - $160.00 for a full 60 minutes  + HST = $184.00

   •  Couple and family sessions - $160.00 for a full 60 minutes + HST = $184.00

   *  Fees - due same day of session, by end of day (Invoices due next day)

HEALTH INSURANCE PLANS

    •  If you have a health insurance plan, I may be able to direct bill the plan for most of the session fee, and you are responsible for paying the fee balance.

   •  I will need your plan number and provider's name when you book a session. I will advise you within 48 hours what is available in your plan.

   •  If I am unable to direct bill the plan (this can sometimes happen) I can issue a formal receipt and you would take that receipt directly to your plan provider for reimbursement of your benefit portion.

PAYING MY FEE, OR YOUR PORTION (if using health insurance)

   Payable in three ways; see below

  2. Pay by PAYPAL (Credit Card and Debit)

  3. Pay by CASH, if seen in person, at end of session. Please bring the exact fee amount
